Domestic Student Eligibility Requirements

Newly Enrolled Students
ALL newly enrolled domestic Florida State University undergraduate students taking 12 credit hours or more and graduate students taking 9 credit hours or more are required to have adequate health insurance. Students must enroll in this insurance Plan or waive enrollment by providing comparable insurance information before orientation. If proof of comparable coverage is not provided to the University by the deadlines listed below, the student's registration will be blocked until proof of adequate insurance coverage is provided or the student enrolls in the university sponsored health insurance Plan:

Fall August 15, 2013, Spring January 1, 2014; Summer May 10, 2014

Part-time Domestic Students
Domestic Florida State University undergraduate students taking at least 6 undergraduate credit hours or graduate students taking at least 3 graduate credit hours are eligible to purchase the Student Health Insurance Plan on a voluntary basis by the enrollment deadlines listed above.

Post Doctorate Fellows and Visiting Scholars
May enroll in the Student Health Insurance Plan on a voluntary basis by the deadlines listed above.

Dependent Eligibility Requirements
Covered Person's may also enroll their lawful spouse and unmarried dependent children under age 19 (25 if in school) who reside with, and are fully supported by, the Covered Person for the same coverage.
