Student Health Insurance Applications and Brochures:
Students may view or print a variety of information. If you decide to print be sure to print all pages of the information you need. You may enroll using the application. Mail the signed application, with a check, to the address located in the application instructions. If the application is not available please contact Collegiate Risk Management.
*IMPORTANT INFORMATION FOR RESIDENT STUDENTS - TIME SENSITIVE
Students that live on campus must have health insurance coverage of some kind. Students that are currently insured under a comparable health insurance plan, such as coverage through a parent’s plan or individual coverage may WAIVE the Basic Student Health Insurance Plan with proof of existing coverage. Resident students must either waive or be enrolled in this plan.
Resident Students must complete the waiver form online by January 20, 2017 or you will be automatically enrolled in the Basic Student Health Insurance Plan and the cost of coverage will be billed with your tuition.

Non Resident Students: For students not living on campus, the Basic Student Health Insurance Plan is optional and you may enroll by downloading an application form.
